Angeles Del Sol ADHC (Adult Day Health Care Center) is a Community Based Adult Services licensed program that provides services to eligible older adults and/or adults with disabilities to restore or maintain their optimal capacity for self-care and delay or prevent inappropriate or personally undesirable institutionalization.
1) As a member of the multidisciplinary team for dual ADHC, will assist Activity Coordinator to conduct initial assessments and reassessments of all participants.
2) Will develop and implement individualized plan of care based on the participant's assessment, only applies to ADHC component of the program. The activity coordinator will involve the participants in the program planning process.
3)Will assist Activity Coordinator complete quarterly progress notes on each participant, only applies to ADHC component of the program.
4) Assist Activity Coordinator to plan and implement all activity programs at the Center.
5) Develop and post a monthly calendar of events.
6) Assist Activity Coordinator to schedule other staff and volunteers for leading and co-leading activity programs.
7) Assist Activity Coordinator to order supplies, keeps records of activity expenses, makes recommendations for supplies, materials, and equipment that will enhance the programs benefit to the participants.
9) Maintain appropriate record keeping and documentation of participants response to activities offered and attended.
10) Participate in staff in-service and other educational and training opportunities. Seek out additional training related to activity programming for persons with dementia.
11) Conform to all applicable policies of the Center and attend the orientation program offered by the Center.
12) Comply with all health and safety standards including licensing, fire and disaster preparedness regulations and agency policies.
13) Attend all staff and other scheduled meetings.
14) Take responsibility for developing activity programming that contributes to and enhances the goals of restoration to self-care and to prevent further deterioration.
Qualifications:
- Fluent in English and Chinese
- Proficient in utilizing computer programs.
- Previous experience in a social, recreational, or educational program providing activity programming in a health care or social service setting desirable.
- Professional disciplines of occupational, art, music, dance, or recreation therapist, occupational therapy assistant or social worker are eligible candidates for this position.
- Activity Assistant is a full time or part time position based on center needs and reports to the Program Director.
Job Types: Full-time, Part-time
Pay: $14.00 - $18.00 per hour
